Then you get the benefits of having things stored on a server with backups and being able to reach the solution from almost everywhere. But you'd have to search. I don't see the necessity to use runtimes because of that reason. Data can stay inside the health facility no matter the platform, technology or tool. This comes down to what has been discussed extensively in these forums do a search on 'runtime' : value for money. FM is not a free platform, if the client cannot be convinced that the solution provided is worth the amount it takes to set up the proper infrastructure including licensing then FM is perhaps not the tool for the job.

I'd spend considerable time trying to convince the client of the value of the solution National Health Service won't permit health data to go outside ie. This sounds to me that the database contains ePHI information and is subject to regulations. If that is the case then that also meant that data needs to be encrypted at rest EAR.
